Proactive Approach

Each incident or accident investigation seeks to determine the immediate cause and contributing factors. 

However, rather than waiting for an accident to occur and then investigating contributing factors, a proactive approach could be taken. 

By identifying and listing all potential contributing factors, an organization could assess if their current policies, procedures, and instructions adequately cover those factors. 

This would allow for the identification and reduction of risk before an incident or accident occurs, taking into consideration the potential consequences and likelihood of occurrence. 

Such a proactive approach can significantly improve safety and reduce the likelihood of incidents, as well as demonstrate a commitment to continuous improvement in health and safety management.
